(October 23, 2012) – Piedmont Medical Care Corporation, the parent company of Piedmont Physicians Group, has been named as the recipient of the 2012 Fred Graham Award for innovation in improving community health by MGMA-ACMPE, a highly sought-after distinction that demonstrates the highest ideals of medical practice leadership.

The Fred Graham Award, which is given to a healthcare organization that has developed activities and solutions to advance the effectiveness of healthcare delivery, was awarded to Piedmont Medical Care Corporation for its chronic care measurement programs. Piedmont Medical Care Corporation’s chronic care measurement programs assess and reward primary care doctors who effectively demonstrate good processes and standards of care – all guidelines that are based on national metrics and standards of care endorsed and utilized by the National Committee of Quality Assurance (NCQA).
Additionally, Piedmont Medical Care Corporation was recognized by MGMA-ACMPE for its partnership with CIGNA and the development of the Patient Centered Medical Home: Collaborative Accountable Care program. This medical home program was designed to improve access to quality patient care and provide better care coordination while lowering medical costs. The program, which targets active patients who are 18 years or older – regardless of insurance coverage – and have diabetes, hypertension and/or coronary disease.
